Adult Volunteers
All forms can be completed by printing the pdf of each form, completing it, and sending it to the address listed on the form. You can also complete the fillable version of the form, provide and acknowledge the electronic signature and submit it directly to us by email, or using the upload form below.
High School Volunteers
Completed High School Volunteer Applications Include:
1. Medical Release (signed by parent)
2. High School Volunteer Application (signed by parent & student)
3. Background Check (signed by student)
4. Mandatory Training Required: Brigid Collins Child Abuse Prevention Training
